[website] Added unban request forum (to keep forums tidy)

Added an unban forum with Approved and denied sub-forums to keep the forums tidy.

Im sick of seeing unban requests littered around without thought, at least then we have an area we can organise and view all requests made, at present its impossible to find existing unban requests if needed.

As a reminder, we don’t do unban requests on PirateCraft, but its inevitable that the posts will be made as thats whats expected from a minecraft server

This forum is classed as a support ticket, its also got a set of rules listed before posting, and it will auto-fill out a forum with information that is needed by the player.

That’ll show up in the ban tracker (When I get it live again) but Ill add it anyway for now 🙂
