You will need to fill out a full form next time you create an appeal however. Your ban reason is as following:

“Used last chance, 7 bans 37 mutes & 12 warnings, you have not learnt to follow our rules, and you are no longer welcomed back.

This comes after the last time you got unbanned from the server where you were given the final chance. You used your chances.
